Service Navigator/Life Planning Coach (Disability Services) (Full Time)

The position reports to the Manager of Case Management.

Job Purpose

Under supervision, to provide a variety of case management services to the disability population, which includes coordinating, monitoring, and providing services, navigating social security, Medicaid, and other social service programs; and to perform related work as required.

Duties and Responsibilities
Conduct Initial and Ongoing Evaluations
  • Conduct initial evaluations to determine the services needed for each potential client.
  • Serve as a client advocate and provide guidance for client self-advocacy.
  • Create and conduct assessments/evaluations as needed.
Conduct Admissions
  • Service Navigator enters admission information into the digital system and complete all admission paperwork.
  • Provide assessments and complete paperwork as related to client needs.
Verify Benefit Eligibility and Application for Benefits
  • Service Navigators verify benefit eligibility for each client.
  • Support clients in making applications for benefits to which they are entitled
Assist Families in Creating Long-Term Plans for Key Elements of Long-Term Wellness
  • Provide support, resources and referrals to assist with decision making around current and future financial needs of the individual with a disability
  • Provide support, resources and referrals to assist with decision making around things like guardianship, advocates, and trustee assignments.
  • Provide support, resources and referrals to assist with decision making around current and long-term housing needs
  • Provide support, resources and referrals to assist with decision making around current and future employment options
  • Provide support, resources and referrals to assist with decision making around current and social and emotional needs
  • Provide support, resources and referrals to assist with decision making around current and future health care needs and decision making
Answer Incoming Calls
  • Answer all incoming calls. This includes responding to questions and explaining the intake process.
Schedule Appointments
  • Schedule evaluations, meetings, and consultations for clients.
Perform Clerical Work
  • Perform various clerical work that includes filing, copying, and faxing.
  • Participating in trainings relevant to disability case management position.
Documentation
  • Document in electronic health record to maintain an accurate record of client services provide.
Qualifications
  • Communication – Excellent verbal communication skills and active listening skills to evaluate clients and assess their care needs
  • Customer Service – Excellent customer service skills to answer questions and guide clients through the intake process, and connect them to local, state and governmental resources.
  • Computer Skills – Must have basic knowledge of Microsoft Word and Excel as well as ability to use Zoom effectively as well as to document and run reports from the electronic health record
  • Time Management –Excellent time management skills to schedule appointments for clients and manage multiple clients at once
Education and Training

Bachelor’s degree in behavioral science, psychology, counseling, social work, or a related field of study required, Master’s degree preferred.

Working Conditions

Ability to work full-time, flexibility in hours including some Sundays and some evening meeting.

Ability to be mobile and work remotely at satellite offices. Possibility of accompanying families/clients to appointments.

Physical Requirements

Must be able to meet routine office physical demands including climbing stairs, carrying up to 20-pound objects and possessing the dexterity and vision to operate office computers and equipment.
